Timing Otoplasty Before School Starts: What Parents Should Know

Otoplasty only takes about a few hours to complete and can be performed once the ears reach adult size, usually around age 5 or 6. If your child has experienced teasing, bullying or other issues related to the shape or size of their ears, this procedure can help restore their confidence just in time for the new school year.

What Can Otoplasty Address?

For children, otoplasty can create a look that feels more balanced and in harmony with the rest of their features. The procedure is tailored to your child’s unique needs and may be used to:

  • Bring prominent ears closer to the head
  • Refine ear shape for improved symmetry
  • Correct differences in ear size or position
  • Address a folded, cupped or unusually shaped ear
  • Create a more proportionate appearance overall

Dr. Payne may utilize different approaches, such as ear augmentation (for underdeveloped ears), ear pinning (for protruding ears) or ear reduction (for large ears). Regardless of the technique used, the goal remains the same: to help your child feel confident in how they look, so they can focus on enjoying life and making memories rather than worrying about their ears.

How to Prepare Your Child for Ear Surgery

It’s normal for your child to feel anxious before surgery, but a little preparation can go a long way toward helping them feel confident and comfortable about their upcoming procedure. As a parent, here are a few steps you can take to guide your child through the process:

  1. Learn about otoplasty. The more you know, the better equipped you’ll be to answer your child’s questions about ear surgery.
  2. Talk to your child about the procedure. Use simple, age-appropriate language to explain what will happen and how it will help them feel good about their appearance.
  3. Highlight the positives and let them know they will still be able to do all the things they love, like swimming or playing sports, once their ears have fully healed.
  4. Stay calm and reassuring. Children often mirror or pick up on their parents’ emotions, so your confidence will help them feel more confident.
  5. Bring familiar items to their appointment. Let your child pick a favorite blanket, a stuffed animal or a beloved storybook to keep with them.

Otoplasty Recovery Considerations

Otoplasty involves a fairly simple and predictable recovery process. Immediately after the procedure, your child’s ears will be covered in bandages, held in place with a head wrap. This should remain in place as directed by Dr. Payne, usually until the next week. The head wrap bandages will be removed in the office with Dr. Payne or his medical assistant.

After surgery, your child will need to wear an earband. We recommend a soft, neoprene band with Velcro straps for ease-of-use, but a wide winter headband will also work, as long as it can be applied without pulling or rubbing against the incision sites. You can use safety pins to help tighten the headband, but be sure not to make it too snug, as that could cause discomfort. Your child will wear this earband 24/7 for at least two weeks to protect the ears and help them maintain their new position. Then, after the first two weeks, the headband will be worn at night only for four more weeks.

Swelling, redness and tenderness are normal in the first week after surgery but tend to subside quickly. Most children can return to school within seven days, with ongoing care taken to avoid bumping or rubbing the ears to ensure proper healing — this means no pullover clothing, rough play or contact sports until cleared by Dr. Payne, usually after six to eight weeks. Once the ears have fully healed, your child will be able to resume all the activities they love comfortably!

Timing Breakdown

  • Day of Surgery: Ears are bandaged and supported with a head wrap.
  • 1 Week Post-Op: Bandages come off and an earband replaces the wrap. Most children return to school with ongoing care to protect the ears.
  • 6 to 8 Weeks: Full healing typically occurs. At this point, children can resume all activities, including contact sports and wearing hats or headphones, comfortably.
  • Scheduling Tip: Aim to schedule surgery a few weeks before school starts. This timing allows for proper healing and a smooth transition back to normal activities. Other popular times for scheduling surgery are before Fall/Winter/Spring break time, so that your child has time to recover before returning to school. We recommend having a consultation with Dr. Payne several months in advance to ensure you obtain the best surgery date for your child, as these times are very popular with our patients.

Tips for Helping Your Child Through the Recovery Process

Helping your child feel comfortable during recovery makes a big difference. Here are some thoughtful ways to support them:

  • Create a peaceful space with soft pillows and quiet activities like reading or drawing.
  • Encourage gentle movement around the house to keep them feeling good.
  • Use the earband as instructed. It may feel unusual at first, but it helps protect the ears and supports smooth healing. Soft, breathable fabrics make it easier to wear.
  • Choose easy clothing that slips on easily without tight necklines. This helps prevent accidental bumps or irritation around the ears.
  • Watch for changes like increased redness or swelling. These signs are worth checking in with Dr. Payne’s office to ensure everything is progressing well.
  • Celebrate milestones such as removing bandages or returning to favorite activities.
